Has anyone found a work around for a home-bound person who cannot sign documents at the bank? I am a co-trustee and durable power of attorney for my 95-year-old dad, but Wells Fargo bank won't let me either take the forms home to him to sign or come to my dad's home. We are trying to change his account to joint ownership so that I can better help him manage his financial affairs. He is not mentally incapacitated (or completely physically incapacitated).

I hope someone comes up with a better answer but this might be a case where you might have to get a Medical Transport and take him to the bank.
If this is possible make an appointment and bring him in.

If this is not possible would they accept a "Zoom" meeting with witnesses in both locations?

WF has a terrible reputation for being difficult but maybe a Notary? Or I like the Zoom idea

Thank you for considering the question. He actually has urinary and fecal incontinence and uses diuretics for heart failure. He's very concerned about having an accident at the bank as well as having the need to continually run to the bathroom. They won't accept Zoom but maybe they would have in-house notaries that could be witnesses. I'll ask.

Take this opportunity to open an account at a smaller local bank that won't treat you or your dad like a number instead of a valued customer.

I'd call around to small banks (or better yet, go in person) to ask how they'd handle that situation. If they'd agree to send someone to the house, just write a check to empty the WF account and fund the new account that way.

If they ark, tell them your dad is happy to talk to them alone to assure them there's no coercion and that he's of sound mind.
