They don't realize how bad it is.

Having your children realize your concern for your health won't help you address your health or get better. You have to take control of your own welfare. Make an appointment with your doctor and discuss your concerns. Maybe take one of your kids with you.

Sorry that you are having sundown syndrome. Are your kids in denial or just not around enough to realize what is going on?

How to tell them? I would just go ahead and tell them the truth and they will just have to grow up and accept the facts and move on from there.

Just be honest. I'm sure this is very hard for you. It will be hard for them to hear as well. Are you on good terms with your children? Do they live near you? Do you live alone? Are you in a facility? Are you scheduled to see your doctor soon or is it possible to make an appointment soon to discuss your situation? Please fill us in a bit more..

I do not have experience with Alzheimer's disease but many on this forum do. They will help you as much as possible. Wishing you all the best. Take care.

Perhaps you can do it in a brief email to each child, with links to explanations of sundowning. That gives them more information without you needing a difficult conversation. They can come to talk to you better informed and less disbelieving. All you need to write is ‘I am fairly sure that I have Sundowners syndrome. Please could you look at this site:

Have you been diagnosed with dementia? Maybe the anxiety you are feeling can be attributed to something else? Have you been tested for covid? It can cause all kinds of neurological problems.

What do your children already know about your health? I see you've logged your post under "Home Care" - is that because you are already being supported to live at home, or because you need more help to manage?

If you can tell us a little more about your situation we can make better suggestions.
