The home should always be assessed before a rehab patient returns home. But, going to a facility? Interesting. Maybe because part of the assessment for MC is whether home is safe? If not, then finding MC may be simplified.. just a guess.

It seemed odd to me as well since the decision to transfer to the MC facility had already been made. A deposit was even paid to the MC facility.

Who at the rehab is telling you about a home assessment?

When my mom was in rehab, the OT told us that they would do a home assessment. We had already determined that mom needed to go to AL, so we turned it down.

During the 1st Care Plan Meeting, the folks at the Skilled Nursing Facility/Rehab said they would be doing a Home Assessment as part of their Care Plan. Since it had already been decided the next step would be a MC facility, I wondered why.

Yes. However, we do have a family member who is trying to have the patient moved to his house. He wants control of her money! That’s why I asked because I didn’t think a home assessment was needed since she wasn’t going home!

No, home assessment is not needed if at discharge being sent to MC. Who has DPOA, because that is the person in charge. If family member doesn't have it, unless he/she seeks guardianship he can't do anything. And if FM gets guardianship he must account for every penny he/she spends and it better be on the person he has guardianship on.

That’s what I thought too! The FM does not have DPOA. However the one who does, has been worn down over the years and the FM is very good at manipulation. We are keeping an eye on things and staying as involved as we can be! I just didn’t know if the Home Assessment by the Rehab/SNF task was something new. I used to attend these meetings in my previous job and was the one in charge when we went down this road with my Grandmother. But it has been many years since both! Thank you.
